Provider and Agency Referrals

We appreciate your willingness to collaborate on patient care.

Our agency’s is happy to receive referrals at our fax number.

We do not reach out to patients when a referral is received. The patient has 30 days to call or visit our office. If we have not heard from them within the allotted time frame, the referral will be destroyed and a new one must be received.

Referrals are NOT required to be seen in our office. If you would like to be seen or join a clinicians waitlist, please do not hesitate to reach out.
